What this website is

This is a marketing website describing a service we are building: an AI voice agent that calls you, builds a medical profile from the conversation, and fills out intake forms on your behalf.

That service is not available yet. Everything on the home page describing what the agent does, what your profile contains, and how forms are delivered is a description of intended functionality, not of a system you can use today.

The early-access form does not book anything

Submitting the form on our home page sends your details to us. It does not confirm an appointment, does not create an account, and does not entitle you to the service. It puts you on a list, and the day and time you pick tell us when suits you rather than reserving a slot.

We may call the number you give us to run through an intake conversation. We cannot promise when.

The call is with an AI, and it is recorded

If we call, you will be speaking to an AI voice agent, not a person. It identifies itself as one at the start. The call is recorded and transcribed so your answers can be written up, and the recording is held by the vendors named in our Privacy Policy. If you would rather not be recorded, say so and hang up — there is no other way to use the service.

While we are testing, do not give the agent real medical information. We have not signed the agreements HIPAA requires with the companies carrying these calls, so the pipeline is not ready for real health details.

Pricing is not an offer

The prices shown on our home page describe how we intend to charge. They are not an offer you can accept, no payment can be taken, and they may change before launch.

This is not medical advice

Nothing on this site is medical advice, and neither IntakeFormHealth nor anything we build is a substitute for a clinician. The agent organises and transcribes what you tell it. It does not interpret it, will not tell you what it means, will not diagnose, and will not make clinical decisions. If you ask it a medical question it will tell you to ask your doctor, and that is the right answer.

You remain responsible for the accuracy of anything you tell us and for checking any form before you sign it. A completed form is a convenience, not a verified medical record. In an emergency, call 911.

Text messages

We text you only if you opt in on our form, only about your own request, and never for marketing. Message frequency varies and message and data rates may apply. Reply STOP to stop, HELP for help. Consent is not a condition of using the service. See our SMS Terms for the full programme description.

No warranty

This website is provided as-is. We do not promise it will be available, error-free, or accurate. Because it describes a product that does not exist yet, parts of it will become out of date, and we may change or remove anything on it without notice.

Limitation of liability

To the fullest extent the law allows, IntakeFormHealth is not liable for any loss arising from your use of this website or from reliance on statements about a service that has not launched.

Governing law

These terms are governed by the laws of the State of Georgia, United States, without regard to conflict-of-law rules.

Changes

We will update this page as the product changes, and move the "last updated" date at the top. Expect substantial changes before launch.
